Complete the Doctor Referral Form
A doctor referral is required for your patient to begin receiving most services at Braille Institute. Just fill out this one-page form.

Return the Form
Once you complete the form, please mail or fax it to the Braille Institute location closest to your patient using the contact information on the form.

Your Low Vision Patient Makes an Appointment with Braille Institute
Your patient can make an appointment by calling the center nearest them directly, or by calling 1-800-BRAILLE (272-4553).
